LINCOLN, Neb. (AP) — Nebraska’s corrections director says two inmates escaped from a Lincoln prison because staff members failed to follow multiple written procedures.

Corrections Director Scott Frakes said Monday that Lincoln Correctional Center employees failed to have shift supervisors present when inmate workers were moved, didn’t seek managerial approval before placing inmates on a work roster, didn’t conduct an informal hourly head count and didn’t properly supervise laundry carts.

Authorities captured 37-year-old Armon Dixon in Lincoln on Saturday, but 52-year-old Timothy Clausen remains at large. Authorities say they received a report Sunday that Clausen was in north Omaha, and they’re searching homes in that area.

Gov. Pete Ricketts says state officials will make available whatever resources are needed to catch Clausen. He says officials will conduct an investigation to learn where security failed.

The office says the pickup rolled several times, ejecting a passenger. He was pronounced dead later at a Yankton hospital. He was identified as 45-year-old Delwin Chavis, of Hudson, South Dakota.

NORFOLK, Neb. (AP) — A woman accused of child abuse in the death of her 4-year-old daughter wants her trial moved out of Madison County.

Lawyers for 21-year-old Carla Montoya last week filed a request for a change of venue. The motion said extensive coverage of the case and online comments by potential jurors would prevent Montoya from having a fair trial in Madison County.

Her trial is scheduled to begin July 11. She’s pleaded not guilty to a charge of intentional child abuse.

Court documents say Montoya told Norfolk investigators that she had tossed her daughter Caylee into her bed three times on March 12. Montoya said she thought her daughter may have hit her head on the bed frame or a wall.

Authorities think the two inmates who fled the prison Friday made their getaway in a laundry truck. One of the men, 37-year-old Armon Dixon, was recaptured Saturday in Lincoln. Authorities say the other, 52-year-old Timothy Clausen, remains at large.

Dixon has been serving up to 278 years for the violent, armed rapes of two women in separate attacks in 2009. Clausen has been serving 50 to 55 years for first-degree sexual assault of a child and tampering with a juror.
